Patellar tendon pain, explained

Why your knee hurts just below the kneecap, and why rest alone rarely fixes it

A 4-minute guide to patellar tendon pain: what is going on, what the research says works, and where a strap actually fits in.

Where it hurts

The pain has a precise address.

Patellar tendinopathy, often called jumper's knee or patellar tendonitis, usually hurts in one spot: where the patellar tendon meets the bottom of your kneecap.

That tendon carries the force every time you land, climb a stair, squat or push off. When the load climbs faster than the tendon can adapt, that spot gets sore, and it often gets worse after sitting, on stairs and at the start of a run.

How common is it

Nearly half of elite volleyball players had it.

A study of 613 elite Norwegian athletes found current jumper's knee in 14% overall, and far more in jumping sports. Those who had it had lived with it for an average of 32 months.

months: the average time athletes in that study had been living with their symptoms. Waiting it out rarely works fast.

Does this sound like you?

Signs it may be your patellar tendon

  • The pain sits just below the kneecap, in a spot you can point to with one finger.
  • Stairs, squats, lunges or landing from a jump bring it on.
  • It eases once you are warmed up, then comes back after you stop or the next morning.
  • Sitting for a long time with your knee bent makes it ache.

This is general education, not a diagnosis. If your knee is swollen, locks, gives way, or the pain followed a fall or a twist, see a doctor or physical therapist first.

What the research says

Rest calms it. Loading is what changes it.

Tendons respond to load. The programs with the best evidence for patellar tendinopathy all use slow, heavy, progressive exercise, while supports help you keep moving comfortably along the way.

Heavy holds can switch pain down, fast

In a randomized cross-over study of 6 volleyball players with patellar tendinopathy, pain during a single-leg decline squat (0 to 10):

Before holds7.0
After holds0.2
Before reps6.3
After reps3.8

Isometric holds vs. isotonic reps. Relief after holds lasted at least 45 minutes. Rio et al., Br J Sports Med, 2015.

25°

Decline squats

Squatting on a 25 degree decline board reduced pain in athletes with chronic patellar tendinopathy in a pilot study, more than the same squats on flat ground.

Purdam et al., Br J Sports Med, 2004.

6

Months later, still better

In a trial of 39 men, decline squat training and heavy slow resistance both improved symptoms and held at six months. Steroid injections helped at first, then faded.

Kongsgaard et al., Scand J Med Sci Sports, 2009.

16

Straps help you keep moving

In 16 young basketball and volleyball players, an infrapatellar strap reduced pain in three of four jump tests, with no loss of jump height.

Dar and Mei-Dan, Prosthet Orthot Int, 2019.

Where a strap fits in

Pressure on one spot, not the whole knee.

A knee sleeve squeezes everything. A patellar tendon strap presses on one place: the tendon just below your kneecap. Imaging research found that infrapatellar straps lowered the strain predicted at the usual pain spot during a modeled jump landing.

  • Contoured 3D silicone pad that sits on the tendon, not on the bone.
  • U-shaped top edge that cradles the bottom of the kneecap, so the pad stays centered while you move. That is the slipping problem most cheap straps never solve.
  • Two straps in every box, one for each knee or a spare for your bag.

Lavagnino et al., Sports Health, 2011 (20 adult men, computer model of strain).

The Knee System

Calm it. Load it. Protect it.

Most knee brands sell you one product and wish you luck. We built the whole routine around how tendons actually recover, and every order comes with The Knee Reset, a free 14-day plan that puts it together.

1. Calm

Cold after training to settle a sore, irritated tendon.

2. Load

Slow holds and decline squats, the exercises with the strongest research.

3. Protect

A strap on the tendon for stairs, runs, lifts and game days.

Who we are

A small brand, out in the open.

Patellar Tendon Strap Co. is a small online brand run by House Lakan Limited in Hong Kong. Your order ships with tracking from our fulfilment partner's warehouse in Jinhua, China, and arrives in 6 to 10 business days. Questions go to a real person at hello@patellartendonstrap.com, answered within 1 business day.

We launched in October 2026, so there are no customer reviews yet, and we won't make any up. That is why every strap order comes with the 90-day money-back guarantee.

Questions

Before you decide

Will a strap fix my patellar tendinopathy?

No strap fixes a tendon on its own. It can make activity more comfortable by changing how load reaches the sore spot. Progressive exercise, like The Knee Reset plan, is what helps the tendon adapt. If pain is severe or lasts more than a few weeks, see a physical therapist.

Strap or kit: which should I get?

If you want something for sport and stairs, start with the straps. If you also want cold after training and a sleeve for warmth, the kit costs $19.90 less than buying the three separately.

How long until it arrives?

Orders are processed in 1 to 2 business days and delivered in 5 to 8 business days with free tracked shipping. Expedited shipping is available at checkout. Orders ship from our fulfilment partner's warehouse in Jinhua, China.

Is it right for Osgood-Schlatter or teens?

Our straps are sized for adults and may be loose on smaller legs. Check with your child's doctor or physical therapist first.
